We are excited to welcome your child to school. As we prepare for the upcoming school year, there are several documents you will need to gather to meet state requirements. This letter is intended to help make your child’s transition into kindergarten as smooth as possible by outlining the necessary steps. Please follow the instructions provided to pre-register your child, and then wait for the enrollment email to complete the official registration process.
On Tuesday, March 10 at 6:00 p.m., there will be a parents-only meeting held in the Wahoo Elementary Cafeteria to discuss your child’s kindergarten experience. This meeting is for parents only, and we ask that children do not attend. A separate session will be scheduled later for students so they can meet school staff and future classmates.
State law requires that when a student enrolls in a school for the first time, the person enrolling the student must provide documentation of the child’s identity and age within thirty days. This can be either a certified copy of the student’s birth certificate or other reliable proof along with an affidavit explaining why the birth certificate cannot be provided. Birth certificates will also be submitted online.
The school must have a certified copy of the birth certificate issued by Vital Records. Hospital certificates are not accepted as official documents. You can request a certified copy from Vital Records in Lincoln, Nebraska, or through the Nebraska Department of Health and Human Services website. This document should be obtained and submitted as soon as possible before school begins.
An immunization record is also required and must be submitted through the Parent Portal before the start of school. Kindergarten students must have the following immunizations: three doses of DTaP with one given on or after the fourth birthday, three doses of polio vaccine, two doses of varicella vaccine given after 12 months of age or proof of the disease, two doses of MMR vaccine given after 12 months of age, and three doses of Hepatitis B vaccine.
Exemptions may be granted for medical reasons with a signed statement from a physician or for religious reasons with a signed affidavit from a parent or legal guardian. Waiver forms are available in the school office.
A physical examination form must be submitted online before the first day of school. The exam must be completed within six months prior to school entry. A written refusal will also be accepted.
An optional dental form may be submitted online at any time.
A professional vision examination is required, or a written refusal may be submitted instead.
All forms can be completed or uploaded online once your Parent Portal account has been set up in PowerSchool.
Thank you for your cooperation. We look forward to meeting you on March 10 at 6:00 p.m.
Sincerely,
Ben Kreifels
Elementary Principal
