By clicking on the links below, you can access the 2008-2009 physical forms and parental consent forms. In addition, the physical forms are available at the local medical clinics, and from the high school and middle school offices. The parental consent forms can also be found in the 2008-2009 high school and middle school student handbooks.
REMINDER: A student must have a physical completed and the consent form returned to the school prior to participation in any athletic activity. In addition, students must pay the $30 activity fee or have a free and reduced form returned to the high school or middle school offices prior to the first contest.
